Management of Eye Flu (Netrabhisyanda) With Ayurvedic Preparations

Abstract Eye flu also known as conjunctivitis or red eye is highly contagious disease. It may cause various clinical features and is also responsible for blindness some times. It may spread from one person to other person through different modes. No standard treatment is available for eye flu presently. Many medications of different systems of medicines are used without providing any long-lasting results. The reason to select eye flu in the present study was to compare it with Netrabhisyanda. Secondly, the goal of the present study is to come out with specific ayurvedic treatment for the management of eye flu. In the present study ayurvedic texts were gone through so as to compare the nidana, lakshans and samprapti of netrabhisyanda with clinical features and pathogenesis of eye flu. Because of the aforesaid reason, chikitisa mentioned for netrabhisyanda was selected for administration to the patients of eye flu. It is discovered through the study that application of anjana karma (collyrium), eyedrops (ascyotana) and external application of paste on eyelids (vidalaka) as mentioned in the ancient texts will produce supradditive potentiated synergistic effect which will act as an important hallmark for curing the eye flu. If these treatment modalities are made a daily routine by healthy individuals who live in sensitive endemic zones to this disease, will develop their immune system against this contagious entity.

Introduction
Eye flu or conjunctivitis, also known as Pink eye is among the most common eye diseases worldwide. It is a dangerous and contagious disease which can spread from one person to another through different modes. The prominent systems are red eye, itching, watery secretion, thick yellow discharge from eyes, burning eyes, blurred vision, photophobia and inflamed eyes. The most common cause of eye flu is viral and bacterial infection. But can be caused by other factors like allergy, UV rays, irritants and certain medical conditions. Application of cold or hot compresses, instillation of artificial tear drops, application of antibiotic eyedrops and ointment are the measures taken to treat the disease. But these are partially effective as these can alleviate the symptoms for sometime and is unable to cure or prevent the eye flu. Thus, there is no therapy which may provide an answer for the management of the condition. Preventive measures include avoiding the touching or sharing items with the persons suffering from disease, which is almost impossible to follow this for long duration in routine life. Eye flu may transmit as endemic or epidemic though different modes. Therefore, taking into consideration, importance and gravity of the disease, in this research work Ayurvedic textbooks were explored to compare eye flu with some roga mentioned in granths and at the same time to cure it. It is discovered through this study that Netrabhisyanda have similar clinical presentation as Eye flu. Moreover, eye flu is selected for this research project as presently no satisfactory prophylactic, conservative and surgical treatment is available for the present ailment. Thus, present study is aimed to select and clinically analyze the drug from the Ayurvedic granths to manage the eye flu which will not produce any side-effects as well. Thus, Darvi rasanjana yoga, Karavirarasa ascyotana, Saindhavadi vidalaka are selected for the present research work.
Aim of study Aims and objectives of the present study 1. To compare eye flu with netrabhisyanda. 2. To evaluate safe and effective treatment from ayurvedic granths for the management of Eye flu (Netrabhisyanda). 3. To know etiopathogenesis and pathology of Eye flu.

It is important to mention here that the present article is different from the above articles as comparative study of Netrabhisyanda is done for the first time with Eye flu in the present research article. Moreover, Ayurvedic remedies were found for the management of Eye flu by the Author painstakingly and to know their mode of action and pharmacological properties of the ingredients mentioned in Ayurvedic granths.

Methodology
1. Knowledge about eye flu was collected and upgraded from the modern textbooks, internet and dictionary. 2. Ancient granths like Cakradatta, Sushruta Samhita, Astanga Hrdaya, Charak Samhita, Laghutriya granths, Bhaishajya ratnavali were studied regarding lakshans (clinical features), samprapti (pathology) and to trace the chikitsa (management) for Netrabhisyanda (Eye flu). 3. Ayurvedic samhitas related to Dravyaguna vigyan and Ras-shastra were reviewed to gain the knowledge about the herbs and ingredients of the remedies mentioned in the samhitas. 4. Textbooks of Pharmacology were undergone in depth to establish mode of action of therapies mentioned In Ancient books.

Selection of Drugs

I.  Darvi rasanjana yoga - (Cakradatta Treatment of Netra roga Chapter 59/7).

II. Karavirarasa ascyotana- (Cakradatta Treatment of Netra roga Chapter 59/8).

III. Saindhavadi vidalaka- (Cakradatta Treatment of Netra roga Chapter 59/10).

Mode of Administration

iDarvi rasanjana yoga - Application as Anjana karma (collyrium).

ii. Karavirarasa ascyotana- Application as ascyotana (eyedrops).

iii. Saindhavadi vidalaka- Application as vidalaka (external application of paste on the lids).

Dosage

iDarvi rasanjana yoga- Anjana karma once in a day (oculus dexter).

ii. Karavirarasa ascyotana- 8 drops twice a day (bis in die).

iii. Saindhavadi vidalaka- External application of paste on the eyelids once in a day

(oculus dexter).

Discussion

Regarding correlation of Eye flu with Netrabhisyanda

Eye flu can be correlated or taken as Netrabhisyanda as the clinical features of the disease Netrabhisyanda mentioned in ayurvedic texts have the same as mentioned for Eye flu-

i. It is mentioned in Sushruta Samhita that generally all diseases of the eye commence with abhisyanda as their chief cause, hence abhisyanda should be treated at its onset itself quickly for well-being of the eye. Similarly, Eye flu is highly contagious disease which can cause various clinical features and may also lead to blindness.

In Netrabhisyanda, there is constant pricking pain, restriction of ocular movement, horripilations, dryness of eyes and flow of tears occur. There may be burning sensation, ulceration and desire of cold comforts; increase of watery discharge. Feeling of heaviness, swelling of the eye, thickening and frequent exudations may be present. Redness and presence of bright red lines in the eye may be seen.

Eye flu is the condition characterized by itching, red eye and watery secretion, thick yellow discharge from the eyes, burning eyes, blurred vision, photophobia and inflamed eyes.

It can be learned through the above discussion that Netrabhisyanda and Eye flu are one and the same disease. The gravity of the present disease/roga can be estimated from the fact it is main reason of almost all the eye disorders.

Regarding treatment

Treatment modalities, their Pharmacological/Therapeutic effects and Mode of Action

Treatment modalities-

I. Darvi rasanjana yoga

 It is made of Daruharidra (Berberis arsitata D.C.) and milk. This preparation should be applied on the eyelids as anjana.

Pharmacological/Therapeutic Effects

Daruharidra has pharmacological property of being caksusya. It is effective in Netrabhisyanda; when applied as paste in netrasoth (eye inflammation). It is effective in case of Naktandhya. Daruharidra has therapeutic effect in case of eye infections.

Rasanjana has pharmacological property of being caksusya. It is effective in netra rogas and is said to be the best for the eyes and diseases of eyes.

Human milk cure diseases of the eyes.

Mode of Action

Application of anjana (collyrium) will remain in contact with eyes for a prolonged duration and high concentrations are attained at the desired site and the absorption of the medication will occur for more time. Moreover, potentiation synergistic effect will increase the efficacy of the medication.  

II. Karavirarasa ascyotana

Karavirarasa ascyotana prepared from tender leaves of karavira should be taken. This medicated preparation when applied will cure the eye flu.

Pharmacological/Therapeutic Effects

Karvira has therapeutic property of being useful in eye disorders as its pakva swarasa is instilled in the eyes for managing eyes diseases. Karavira has therapeutic effect in case of Netrabhisyanda and Netrapaka.

Mode of Action

Application of ascyotana will help to cure the disease as the medicated eyedrops will remain in contact with eyes for prolonged period and due to its liquified nature will neutralize and remove the toxins produced by causative organisms. 

III.Saindhavadi vidalaka

Saindhavadi vidalaka prepared from Saindhava (Rock salt), Daruharidra (Berberis arsitaita), Gairaka (Red ochre), Haritaki (Terminalia chebula) and rasanjana when applied to the eyes as vidalakya will cure the eye flu.

Pharmacological/Therapeutic Effects

Saindhava has a therapeutic effect as caksusya.

Gairaka (Red ochre) has pharmacological property of being caksusya and therapeutic property being Netrya.

Haritaki (Terminalia chebula) has pharmacological property of increasing efficiency of cakhsu indriya. It is applied on the eyelids for treating Netrabhisyanda. It is administered for curing weakness of eyes. Haritaki has therapeutic use in eye diseases. Its cold compresses are very effective and useful for eyes.

Mode of Action

Application of vidalaka will remain in contact with eyelids as well as with the eyes for prolonged duration and the absorption of the medication will occur for increased time period. Efficacy of the medication will be further upgraded due to synergistic potentiation effect of the preparation.

Conclusion It is concluded from the above discussion that Eye flu and Netrabhisyanda have same clinical features. The symptoms of the two diseases are totally similar and same. Thus, these two conditions are correlated with each other and it is concluded that these should be taken as comparative diseases as per the discussion done above. The treatment remedies of this research work will be effective in the management of Eye flu. Anjana (Collyrium), Ascyotana (Eyedrops), Vidalaka (External application of medicated paste on eyelids) are efficient to cure Eye flu. As collyrium prepared with medicated herbs will provide relief through synergistic potentiation effect and thus increasing efficacy of the medication and will prevent/cure the Eye flu. Ascyotana (Eyedrops) will provide relaxation and nutrition to the eyes and thus will be be helpful in the treatment of disease. Vidalaka (External application of medicated paste on eyelids) will cure the eye flu through its mode of action as explained earlier. Thus, it is concluded that the medications indicated for Netrabhisyanda are effective in the management of Eye flu either as prophylactic measure or cure. These will cure the symptoms as well as signs of the disease and will prevent the occurrence of Eye flu correspondingly. All the medications as discussed above have supra-additive synergistic potentiation effect. Further, the mode of action of these therapies is based on the principles of pharmacodynamics of Modern Pharmacology.
